This proceedure is going to depend on which type of Blackberry you have. You are going to have to download modem scripts in order to make this happen, and different Blackberry's require different scripts.
Curve (8300 series) (The firmware must be 4.5 or later) And the scripts can be downloaded off of perishable press. http://tinyurl.com/bb8300
Pearl (8100 series)

Begin pairing process on computer by clicking the Bluetooth icon in upper right hand corner and selecting Set up Bluetooth Device
Click Continue after Bluetooth Setup Assistant opens.
For the Device Type choose Mobile phone and press Continue.
Highlight the name of the device and press Continue
The assistant will gather information about the device. Click Continue.
A Passkey will now be displayed. Enter it into the device and the screen will advance to the next. If it doesn't press Continue.
A list of services will now come up. The only one needed is Access the Internet with your phone's data connection. Ensure that Use a direct, higher speed connection to reach your Internet Service Provider (GPRS, 1xRTT) is selected. Press Continue.
Enter the Dial Up Network (DUN) specifics in the next screen. The Username and Password is [gprs].
The GPRS CID String is the provisioned APN. Check the phone's TCP section to determine which APN to use. This example requires [epc.tmobile.com].
Set the Modem Script to Blackberry 3G CID1.
Checkmark the boxes that say Show Bluetooth status in the menu bar and Show Modem status in the menu bar. Press Continue.
Press Quit to finish the assistant.
Click on the Apple menu icon in upper left hand corner and choose System Preferences.
Click on Network.
Change the Show drop down menu to Bluetooth.
The PPP tab will then appear. Most fields will already be filled out. The last field to populate is Service Provider. Enter that as T-Mobile Bluetooth to indicate network and type of connection.
Click PPP Options
Uncheck all boxes here and then click OK.
Proceed to the Bluetooth Modem tab. Uncheck Enable error correction and compression in modem.
Uncheck Wait for dial tone before dialing. Click Apply Now.
Click on the Modem icon in the upper right hand corner. Make sure that T-Mobile Bluetooth is checked (this step may be skipped if T-Mobile Bluetooth is not present) and click Connect.
Open Safari and test the connection by browsing.
